Services
Our purpose at Guiding Hands Coalition is to be the leading provider of transitional care services in our community. We do this by providing compassionate, supportive care to each individual client, enabling them to remain active and integrate into their community.
The transitional care services that we provide, but are not limited to, include:
Housing
We locate and coordinate appropriate housing such as Assisted Living Facilities, Adult Family Homes and Apartments for clients.
Federal Rental Assistance
We assist with the application process for federal rental assistance, as well as completing all lease documents with clients. This includes guiding client to make rental and utility deposits.
Social Security Benefits
We assist with the application for social security benefits as well as with the transportation to and from social security office.
Transitional Services
We coordinate clients’ physical move, along with their belongings from skilled care facilities, nursing homes, apartments, houses, institutions or other settings into a new home.
Housing Stabilization
We assist with the purchase and delivery of household medical equipment, as well as assist with and ensure that medical resources are available and met, including location of pharmacies, primary care physicians and care providers.
Atypical Daily Activities
We connect clients with less formal activities, such as YMCA, AA, Senior Community Centers, libraries, local bus systems and local church groups.
Problem Solving
We coordinate care giving and bridge connections for the client with a care provider facility, case managers, medical personnel, legal representatives, family members, informal support, and much more.
Notary Services
We facilitate Notary services as well as other document services.

Transportation Coordination
We transport client to various government facilities and assist in the application process for services. We assist with applications for local transportation such as Dial-A-Ride, and assist and train on using the public transportation (bus) system. We can also assist with applications for vehicle handicap certification.
Shopping
We transport client to go shopping for essentials items at furniture stores or shopping centers so the client can pick out furniture and household goods as necessary ranging from a bed to a shower curtain or even silverware.
Citizenship & Immigration
We assist in acquisition of lost or stolen United States Citizenship and immigration documents. Services for translation of legal documents are also available.
